The day of your appointment
Please arrive 15 minutes early for registration. We will ask you to complete forms and verify that your insurance information is correct. Before your appointment, we will also need to collect a copay or deductible.
Please remember that your appointment time is valuable. If you cannot keep your appointment, let us know as soon as possible so we can offer your spot to another patient. If you do not cancel your appointment in advance, you will be charged a $25.00 no-show fee.
